This is the first step in our enrollment process. Once you have completed this form, your information will be sent to the appropriate site coordinator, depending on the day you select in this form.

If the program is a good fit for your family, they will give you information regarding next steps that will guide you through the enrollment process. Filling out this form does not commit you to enroll in the Options program, nor does it automatically enroll your student(s) in the Options Program.

Important Information Regarding 504s/IEPs

Students with Disabilities: Homeschooled students, including students accessing the Options Program, are not entitled to access special education services, including specialized instruction, or related services pursuant to an IEP. If your student needs this level of support, it exists at your neighborhood school. However, students with physical or mental impairments may be referred for an evaluation under Section 504 to determine if the student qualifies as a student with a disability under Section 504 and whether the student needs accommodations to access CCSD programs, including the Options programs. Students will be evaluated under Section 504 in collaboration with parents, teachers, and other appropriate staff. Please contact us for more information.
